ATHLETIC PARTICIPATION WAIVER AND RELEASE OF LIABILITY.

BY PARTICIPATING, YOU AGREE TO THE ATHLETIC PARTICIPATION WAIVER AND RELEASE OF LIABILITY.

YOU PLAY AT YOUR OWN RISK.

In consideration of being allowed to participate in any way in REVOLVER FC, AMERICAN SPORTS MGMT, SCOUTING PROS, OR AFFILIATES AND COACHES athletic sports program, related events, and activities, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS REVOLVER FC, AMERICAN SPORTS MGMT, SCOUTING PROS to their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

THE BOOKING FEE IS NON-REFUNDABLE FOR ANY REASON.

COVID 19 RISK ACKNOWLEDGE AND RELEASE OF LIABILITY.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that REVOLVER FC AND AMERICAN SPORTS MGMT has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that AMERICAN SPORTS MGMT & SCOUTING PROS can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other participants and their families.

I voluntarily seek to participate in the events provided by AMERICAN SPORTS MGMT & SCOUTING PROS and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending this event.

I attest that:

* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days.

* I have not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/Covid-19 and have not yet been cleared as non-contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the staff, or that may otherwise arise in any way in connection with any services received from REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS. I understand that this release discharges REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS from any liability or claim that I, my heirs, or any personal representatives may have against REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from or participating in the events organized by REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS. This liability waiver and release extend to REVOLVER FC, AMERICAN SPORTS MGMT & SCOUTING PROS together with all owners, partners, and staff.

MEDIA CONSENT, RELEASE, AND WAIVER

I hereby give consent to AMERICAN SPORTS MGMT, SOCCER SCOUTING PROS, PARTNERS, AND AFFILIATES (the “organization”) to photograph, videotape, or otherwise digitally record and use images and/or sound recordings of myself or my child or children (if applicable) to use in any public media, including radio, television, internet, social media, print or in any of the organization’s or its partners’ publications, productions, or posts. I understand that the intended use of such images and information is solely for the purpose of advertising, marketing, fundraising and/or promotional and public awareness purposes for the organization. I hereby waive any rights or interest in the images or recordings as contemplated in this release. I acknowledge that this consent to use images and/or recordings is being made solely for the benefit of the organization and comes without any expectation of monetary compensation or other benefits to me. To the extent that any benefit accrues or might accrue to the organization from the use of images or information, I hereby and forever waive any interest in or claim to such benefits. I hereby release and forever discharge the organization (including without limitation all corporate affiliates and officers, directors, trustees, donors, employees, agents, and volunteers) from any and all claims, liability, actions, suits, demands, costs, expenses, or indebtedness arising out of, related to, or in any way connected with the use of images and materials described herein. I hereby waive all rights and interests in and to such information and materials. I further acknowledge that there is no guarantee that any or all of the participants’ images or recordings will be used in any released media.
